Insulin Resistance in Midlife: The Shift Behind the Scale
The most common story I hear is not that eating changed. It is that nothing changed, and the body responded differently anyway. That is usually insulin, and it is measurable.
What insulin resistance actually is
Insulin moves glucose out of the blood and into muscle and fat. When cells respond less readily to that signal, the pancreas compensates by producing more insulin. Blood sugar stays normal for years while insulin quietly climbs. That compensated stage is where most midlife women are, and it is invisible on a standard fasting glucose test.
High circulating insulin is not a neutral state. It promotes fat storage, particularly around the abdomen, suppresses fat breakdown, drives the mid afternoon energy collapse, and contributes to the hunger that arrives ninety minutes after a carbohydrate heavy meal.
Blood sugar is the last thing to become abnormal. By the time fasting glucose rises, the process has usually been underway for years.
Why perimenopause accelerates it
Estrogen supports insulin sensitivity and influences where the body stores fat. As it declines, fat redistributes from hips and thighs toward the abdomen and around the organs. Visceral fat is metabolically active and worsens insulin resistance further, which is a self reinforcing loop.
Muscle mass is the other half of the picture. Muscle is the largest site of glucose disposal in the body, and it declines steadily from the fourth decade unless it is deliberately maintained. Less muscle means less capacity to clear glucose, at exactly the moment insulin sensitivity is falling.
Signs worth noticing
- Expanding waist even when overall weight is stable.
- Energy collapse after meals, particularly in the afternoon.
- Strong carbohydrate cravings and hunger returning quickly after eating.
- Darkened velvety skin at the neck, underarms, or groin.
- Rising triglycerides with falling HDL on a lipid panel.
What to measure
Fasting glucose alone is insufficient. HbA1c gives a three month average. Fasting insulin alongside glucose allows a calculated estimate of resistance. A lipid panel, and specifically the triglyceride to HDL ratio, is informative. Waist circumference is a better tracking measure than weight for this particular problem.
What reverses it
Resistance training is the single highest yield intervention, because building and preserving muscle directly increases glucose disposal capacity. Two to three sessions weekly is enough to change the trajectory.
Protein at every meal improves satiety and supports muscle. Walking for ten to fifteen minutes after eating measurably lowers the post meal glucose peak. Sleep matters more than most people expect, since a single week of restricted sleep reduces insulin sensitivity in healthy adults.
Where lifestyle change is not enough, medication is a reasonable and evidence based next step, and that conversation should be had before the diagnosis becomes diabetes rather than after.
